Through this program, offered in partnership with our 1199SEIU Benefit Funds and InquisitHealth, home care workers who are successfully managing their own diabetes are trained to coach, advise and share insights with peers who need support.
Mentors complete a web-based training program and are matched with peers who they speak with twice a month for six months. Together, they set health goals and discuss ways to achieve them. InquisitHealth’s online platform is used to prompt calls and send follow-up text messages to remind participants of their goals. The program has shown a positive impact, with 75% of participants who were mentored for more than six months experiencing a drop in their HbA1c levels.
